Please download Consent Form, sign it, and bring it into the office at your initial weigh in.
Schedule Your Weigh In!
Call the office at 770-429-9733 in order to schedule your weigh in. The weigh in is the first step to being a part of the program.
Weigh in once per week, same day, same time... You choose the time and day!
Learn Points Program
- 15 minutes of Walking 1pt
- 15 minutes of Running 2pts
- 15 minutes of Bicycling 2pts
- 15 minutes of Aerobic Activity 2pts
- 5 minutes of Meditating 1pt
- Keeping a Diet Diary 1pt per day
- Drinking Your Water Daily 1pt per day
- Maintaining your Office Care Plan 5pts per week
The goal is to reach at least 50 points per week.
